25 | On 10/05/22, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Case Management Health Check visit. LPA was screened from window for COVID-19 symptoms and completed questionnaires. LPA stated purpose of visit and allowed entrance.
LPA met with Executive Director, Sarah Ehret and Administrator, Sharon Fay and toured facility. Currently, two residents on Hospice. Residents observed to be having lunch, resting in their bedrooms, or participating in afternoon music.
LPA Medina toured kitchen with Director of Dining, Robert Kershner and observed facility to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available. Facility receives food deliveries 2 to 6 times weekly for fresh produce, dairy, meat and poultry and canned goods. All common areas have adequate seating available.
Adequate staffing is available. The facility has an adequate supply of Personal Protective Equipment (PPE) on site which incudes: masks (surgical and N95), gowns, face shields, gloves, bio hazard, and disinfectant wipes.
LPA received copy LIC 9020 (Register of Facility Clients/Residents), monthly news letter to Forest Hill Family, Residents and Staff dated October 2022, LIC 500 Personnel Report dated 9/12/22 and is accurate, LPA also received current schedule dated October 2-8, 2022.
No deficiencies observed during today's visit.
