25 | Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced visit today for the facility’s annual inspection. LPA met with Facility staff, Genovera Bose Continual Administrator's Certification for Cristina Del Rosario expires 06/23/2025.. There are currently 6 residents who reside at this home 0 residents currently on hospice.
LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, garage and outdoor areas. There is a locked storage for medications.
Staff 1 is not fingerprint cleared. Staff 2 does not have first aid/CPR certification. Resident 1 does not have a signed Admission Agreement in file. Resident 2 does not have any identification in file. LPA observed facility bathroom trash with no lid. LPA observed the facility does not have sufficient 2 day perishable food supply, and 7 day non perishable food supply. LPA Hurt observed several facility canned food items to be expired, and several dry food items (crackers) also expired. LPA observed facility window screens are ripped and in disrepair.
The following deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22.
LPA requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. Listed documents shall be sent to Licensing.
