Licensing Program Analyst (LPA) Elizabeth Larios met with Preschool Program Coordinator, Denise Noel for a Case Management Inspection. Preschool Program Coordinator self reported to the Department a unusual incident that took place on 04/11/2024 involving a preschool children (C-1). LPA interviewed staff and children.

Based on interviews there is no evidence to prove that C-1's incident was due to lack of supervision. Since incident items have been removed and will no longer be used for activities. Moving forward staff will assure items used are age appropriate.

LPA conducted an exit interview with Denise Noel where this report was reviewed and copy was provided.
No deficiencies issued. A notice of site visit was given and must remain posted for 30 days.
