Licensing Program Analyst (LPA) Elizabeth Larios conducted an unannouced inspection to deliver the complaint allegation listed above. LPA met with Teacher, Indya Hawthone and explained the purpose of today's visit.

Based on LPA interviews which were conducted, and indoor and outdoor tour, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.

Type B deficiency was cited on the attached LIC 9099-D.

Exit interview was conducted, where this report was reviewed and discussed with Indya Hawthone. A copy of this report was also provided. Appeal rights were given

This requirement was not met as evidenced by: Based on tour observation indoor and outdoor of the toddler & preschool, LPA toured outdoor play areas and observed children's bikes, cars, waffle blocks, play house, chairs, easel, and broken toys, one play structure was rusted in several places, a part of the play ground structure was removed leaving behide a piece of concrete with screws sticking out, and uneven walking surfaces tripping hazard to children in care (toddler & preschool outdoor area). LPA observed disrepair items were not placed inaccessible to children in care. LPA toured the preschool classroom and observed broken blinds, dusty ceilings & indoor play area stairs dusty, sprider webs, rip carpet, dirty rug, stain walls & chip paint, a lose baseboard in children bathroom. LPA observed rip napping mats, rip carpet, stain walls & chip paint in the toddler classroom. This poses a potential risk to the health and safety of children in care.
