25 | Licensing Program Analysts(LPAs), Teodoro Trujillo and Elizabeth Larios, met with Sandra Fletes, Site Supervisor, for an unannounced case management inspection in response to a self reported Unusual Incident that the Facility reported to the Department on August 11, 2022.
On July 29, 2022 a parent reported her child was hit in the hand by a teacher during diaper change.
LPAs were provided with investigation documents that the center conducted. Site Supervisor stated staff (S1) was placed on administrative leave while an investigation was conducted by the center. The conclusion of center investigation was found to be unsubstantiated.
LPAs conducted additional staff and children interviews who were present during the incident. The staff (S1) and child (C1) were not available for interview. LPAs obtained staff and children records. Based on the available information, further investigation is required and shall be continued on a later date.
Exit interview was conducted with Site Supervisor, Sandra Fletes and Administrative Program Director, Dina Lozoya,
No deficiencies were issues during today's inspection.
A notice of Site Visit was issued and must remain posted for 30 day. |
