32 | Site Supervisor self-admitted that the child (C1) was left in the library(Room 16), without any teacher being present for about an hour and half as child (C1) was sleeping in the library(Room 16) during morning after child was dropped off at the facility, on May 12, 2025. Based on interviews, staff left a child(C1) unattended in library(Room 16) without any teacher being present on May 12, 2025 during morning time. Based on interviews, four(4) children(C2, C3, C4 &C5) were left unattended in the bathroom on May 30, 2025. Site supervisor stated that they are not aware of the time duration of four children(C2, C3, C4 &C5) were being left unsupervised in the bathroom. Therefore, the above allegation is SUBSTANTIATED, meaning the allegation is valid because the preponderance of the evidence standard has been met.
During today's investigation, one "Type A" deficiency is issued on the attached 9099-D. Exit interview conducted and report was reviewed with Director, Nitaya Robinson and Site Supervisor, Ashley Reyes. Appeal rights was provided.
A notice of site visit has been issued and must remain posted for 30 days.
LPA informed Director and Site Supervisor that this report dated 06/26/2025 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Assembly bill 633 was provided and discussed with Director and Site Supervisor. LPA informed Director and Site Supervisor to provide a copy of this licensing report dated 06/26/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9224), must be placed in the child's file for verification. |
