25 | Licensing Program Analyst (LPA) Elizabeth Larios & Albert Mendoza met with Licensee, Maybelline Rosado for an unannounced Case Management - Annual/Random inspection. LPA was granted access to the home by the Licensee. LPA observed three day care children in the home during today's inspection. Licensee was operating within her capacity and ratio requirements. LPA observed the required postings, including the facility license, near the front entrance to the home. Days and hours of operation are Monday - Friday from 7:30 AM to 5:30 PM. Licensee and spouse are the only adults residing in the home and there two minor children.
LPA observe a current Child Care Facility Roster, and Fire/Disaster drill log. Licensee & Assistant, Zaida Rosado have current CPR and First Aid certifications. LPA reviewed two staff files (Licensee & Assistant) in prior visit and the files were incomplete with the required forms. Licensee & Assistant had the required vaccines (MMR, Tdap, & flu) but did not have a current Mandated Reporter Training for Child Care Workers Certificate in file. LPA reviewed seven children's files and the files were complete with the required forms. Licensee does not have liability insurance for the day care and issues the Affidavit Regarding Liability Insurance (LIC 282) form to all enrolled families.
LPA toured the indoor and outdoor areas of the home in prior visit. Licensee has a working telephone in the home. The home is clean, orderly, (including heating/fans/ventilation), and safe for the day care children. There are safe & age appropriate toys, play equipment, and materials for the children in the home. There are stairs that were observed barricade and a fireplace in the living room that was also barricade. The off-limit areas: entire second floor, and in the first floor, hallway closets, kitchen, and garage. Off limit areas outside the entire backyard until further notice.
LPA observed a fully charged 3A40BC fire extinguisher last service on 09/27/2024, working smoke/carbon monoxide detector, no bodies of water, and fenced backyard. The Licensee states she does not have any pets or weapons in the home. All detergents, cleaning compounds, medications, and other similar items are inaccessible to children and stored in the locked garage. Licensee has a first aid kit in the home. |
