Licensing Program Analyst (LPA) Elizabeth Larios conducted unannounced Case Management-Incident inspection and met with Candace Frazure Director. Purpose of today's inspection discuss an unusual incident that Maria Assistant Center Director self reported to the Department on April 23, 2024 involving an infant (C1) . LPA observed two teachers, one teacher assistant, eleven infants in Room 3.

During the investigation process, LPA toured the facility, conducted interviews, reviewed camera footage, and obtained copies of documents. Based on the information gathered during the investigation, there is no evidence to prove that infant was injured while in care by a staff or a peer.

Exit interview was conducted with Candace. No deficiencies issued during today's inspection.

Notice of site visit was issued and must remain posted for 30 days.
