THIS IS AN AMENDED REPORT
Licensing Program Analysts (LPAs) Harsimran Kaur and Janette Cruz conducted an announced pre-licensing inspection. LPAs met with Sherilyn Napoli, Applicant/Executive Director and explained the reason for the inspection. The purpose of this inspection: Applicant is applying for a single license with 12 preschool children (ages 3 to 5 years) and 65 school-age children (ages 5 to 16 years). The center is located in the Carmel Youth Center Gymnasium and hours are from Monday- Friday, 7:00 AM to 6:00 PM. Applicant stated that cafe/TV room area shall be used by school age children during transition time of preschool and school age children from 3PM - 6PM.

Applicant advised that there should be a designated area for required postings, such as notification of parent's rights, personal rights, daily activity schedule, facility sketch, car seat law, and menu. There is a working phone at the site, which is (831) 624-3285.
The following are indoor measurements and information:
School-age program
The Indoor total measurements contain a total of {3498.22} square feet, which will accommodate Applicant’s request for 65 school-age children.
Outdoor total measurements contain a total of {2271.17} square feet, which will not accommodate Applicant’s request.

Preschool Program
The Indoor total measurements contain a total of {2392.29} square feet, which will accommodate Applicant’s request for 12 preschoolers.
Outdoor total measurements contain a total of {2271.17} square feet, which will accommodate Applicant’s request for 12 preschoolers.

There are 15 chairs, 3 tables, 34 cubbies, 7 cots, 2 urinals, 5 toilets, and 5 sinks for the preschool program. There are 75 chairs, 7 tables, 34 cubbies, 2 urinals, 5 toilets, and 5 sinks for the school age program. There are no age appropriate fixture and equipment available for children during todays inspection. inaccessible. LPAs also discussed that minimum temperature in the room, which is 68 degrees Fahrenheit. There is fully charged fire extinguisher, smoke detector, and carbon monoxide detector. There is a first aid kit, which has all the required items. Sick children will be isolated in the facility office and isolation equipment will be provided. LPAs observed that facility has theater area for viewing activities. Facility has three meeting rooms that are currently off-limit to children.

The outdoor space is fenced. All meals and snacks will be brought by children from home. There is area to keep food warm and store any perishable food and drinks. Drinking water is provided through water bottles. LPAs discussed that they have 180 days after licensure to receive lead testing for drinking water. LPAs discussed that trash can for waste need to have a tight fitted lid.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. LPAs reviewed with Site Director the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

The following item's needs to be corrected.

1) Proof of available indoor and outdoor fixtures, equipment and supplies.

2) Repair on uneven ground surface located at the outdoor yard.

3) Repair of chipped wood on bottom of storage shed located at the outdoor yard

4) Outdoor shaded area for children.

5) Repair of damaged sink counter on girl's bathroom

6) Waiver request for school age outdoor area capacity of 65 children

7) Proof of portable sink as additional sink.

Exit interview conducted and report was reviewed with Sherilyn Napoli, Applicant/Executive Director. Copy of the report was provided. Final license determination will be made upon review by Licensing Program Manager and correction of the above issues.
