Licensing Program Analyst (LPA) Mindy Mohr made an unannounced Case Management visit to the facility and met with site supervisor Mikali Rinehart (S1). On 07/17/2024 the Department received an Unusual Incident Report from S1, reporting that on 07/10/2024 around 4:00 pm, an unknown woman walked onto campus. The report states a woman approached the children through a chain link fence and started to speak with them. Teachers stepped in and asked the woman to leave. The woman then walked around to the front of the school and continued to talk with the children and staff. Staff then brought the children inside the classrooms and called law enforcement. Law enforcement arrived and removed the woman from the campus.

During today's visit LPA toured the play yard, interviewed S1 and one other staff member (S2). LPA observed 27 children in care being supervised by 5 staff. S1 showed LPA where the woman approached the children, which is fully fenced by chain link fencing. S1 stated no children or staff were harmed during the incident and they immediately informed the parents of what occurred. S1 also stated the facility had a staff meeting to discuss the incident and will be making some changes to replace the back fence to be wood slats and not chain link fencing, as well as utilize walkie talkies as a way of communication for staff who are outside with the children and staff who are inside the classrooms with children.

There were no Title 22 deficiencies cited during today's inspection.

This report was reviewed and discussed with Site Supervisor, Mikali Rinehart.

Notice of Site Visit shall be posted for 30 days from today's visit. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
