| · Licensee Brenna Roth and Director Tamara Kinman (D1) shall complete a 12-hour course in child appropriate discipline and supervision techniques, within the first 60 DAYS of probation.
· Licensee Brenna Roth shall attend courses and training that include the following subjects; operation of a licensed childcare facility, positive intervention and counseling techniques, a course related to stress intervention/anger management and shall engage is support for childcare professionals.
· Respondent licensees and employees shall participate in a minimum of eight hours (8) of annual training, in addition to other mandatory training imposed by licensing laws and regulations, for all staff. Initial training shall occur within four months of the effective date of this Stipulation and shall include training on compliance with regulations, including personal rights and reporting requirements consistent with the obligations of Title 22, and shall also include training focusing on child development, discipline, and behavioral issues. All newly hired employees shall receive the above described eight hours (8) training within 60 days of commencing or returning to employment.
LPA and LPM discussed and reviewed the Stipulation and Waiver; and Order. A copy of the Stipulation and Waiver; and Order must be posted just inside the main day care entrance(s) for parent viewing.
During the inspection the facility was toured inside and outside. LPA and LPM observed 8 children in care. LPA and LPM did not observe a copy of the Stipulation and Waiver; and Order posted. LPA and LPM advised D1 that a copy of the Stipulation and Waiver; and Order shall be posted just inside the main day care entrance for parent's viewing, parents shall sign LIC 9224 and signed copies shall be kept in children's files. LPA and LPM further advised, D1 the facility shall obtain, install and maintain video camera recording equipment for the interior and exterior areas of the facility, wherever children are present.
D1 understands that violation of any of the terms and conditions of probation relating to its stay of license revocation shall constitute sufficient grounds for the revocation of the probationary license. LPA and LPM provided D1 a copy of the Stipulation and Waiver Order and LIC 9224. All licensing reports are public information and must be made available upon request for at least three years.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Exit interview conducted and report was reviewed with the Director, Tamara Kinman.
There were no Title 22 deficiencies cited during today's inspection.
