On 12/11/2025, at 1:20PM Licensing Program Analyst (LPA) Anna Chan conducted an unannounced Complaint investigation inspection. This is to deliver the findings of the investigation initiated on 11/7/2025. Upon arrival, the LPA met Director Rachel White and informed them of the purpose of visit Census was taken. LPA observed 4 preschool children and 1 staff. Children were napping when LPA arrived

A review of the Facility Personnel Report Summary conducted on today’s date indicates all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions.

The Department received a complaint on 11/04/25 alleging
(1) Staff did not ensure daycare was free from pests.
(2) Staff do not ensure daycare is clean and safe

During the course of investigation, LPA interviewed 2 staff, 5 children, 3 parents, and reviewed facility files.
Regarding allegation (1) Staff did not ensure daycare was free from pests.

Reporting Party (RP) stated there have been rat/mouse droppings in every room at the preschool in full access to the children.

During the staff interview, director stated they were made aware of the rat droppings on October 23, 2025, and called pest control immediately. Director stated they closed 2 classrooms, and they use multipurpose room for children's activities. Staff 2 (S2) stated they saw rat droppings in the classrooms. S2 also stated they have been putting on quite a few mouse traps outside and in classroom, and children mainly being outside at the side or front yard for activities.

On 11/7/25 PA observed rat droppings in room 1, 2, 3, multipurpose room, kitchen and storage room. LPA also observed mouse traps in the multipurpose room, and Room #3 closet. Children were in the multipurpose room and rat droppings were visible and accessible to children.



Based on record review, the facility showed proof of Pest and Termite Control from 1st Stryke Pest and Termite Solutions Invoice with service date 10/16/25 for Rodent Proofing “Closing exterior entry points for rodents”. Director stated they called pest control on 10/16/25, since they saw evidence of rodents on the property. And then eventually on 10/23/25, report was sent to the director about seeing droppings in the classrooms and follow up pest control visits after 10/16/25 was conducted.

During children interview, Child 1 (C1) stated they have seen mouse poop in the lunch room.



LPA interviewed parents, 1 of 3 parents expressed concerns about the rat infestation at the facility.

Regarding allegation (2) Staff do not ensure daycare is clean and safe

RP stated facility has done nothing to make sure the school is clean and fit for use.

During the staff interview, director stated they hired a third-party cleaning company to do deep cleaning. Director stated cleaning custodian did not arrive to do cleaning on Thursday (11/6/25) that is why during 11/07/25 complaint visit by LPA; rat droppings were still visible in the classrooms. S2 stated the new cleaner does a better job than the previous cleaner.

Based on LPA’s observations, Room 1, 2,3 multipurpose room, kitchen and storage room all have rat droppings that was not cleaned.



On 12/2/25 LPA observed rat droppings in classroom #3 still on the toy chest.

LPA interviewed children, none of the children interviewed disclosed information that supports the allegation.

LPA interviewed 3 parents and none of the parents interviewed disclosed any information that could supports the allegation.

Based on interview and record review, the preponderance of evidence standard has been met, therefore the above allegations on Staff did not ensure daycare was free from pests and Staff do not ensure daycare is clean and safe, are found to be SUBSTANTIATED. See LIC9099D for 2 Type B violations.


Exit interview was conducted with Director Rachel White. Report and deficiency were discussed. The Notice of Site Visit was posted and must remain posted for 30 consecutive days. Appeal Rights were provided.

During the course of investigation, LPA interviewed 2 staff members, 5 children, 3 parents, and reviewed facility records.

Reporting Party (RP) stated the janitor left dangerous materials, such as cleaning supplies and leaf blowers, in reach of the children.

During staff interviews, 2 of 2 staff stated the janitor would leave cleaning supplies such as bleach, sanitizing spray, leaf blower and vacuum reachable to children. 2 of 2 staff stated they would put away cleaning supplies and bleach when they open the school and before children arrive the school.


On 11/7/25 and 12/2/25, LPA did not observe hazardous items like cleaning supplies and bleach on the premises.


LPA interviewed 3 parents and none of the parents interviewed disclosed any information that could support the allegation.

Based on observation and interviews conducted, the preponderance of evidence standard has not been met. Although the allegation may have happened or are valid, there is not a preponderance of evidence to prove Hazardous items are accessible to children in care violations did or did not occur, therefore the allegation is UNSUBSTANTIATED.



Exit interview was conducted with Director Rachel White. Report was reviewed and discussed. The Notice of Site Visit was posted and must remain posted for 30 consecutive days. Appeal Rights were provided.

Director stated there are no more sightings of rodents or no new rat droppings. The facility has provided Pest Control service documents to LPA. Director stated they will provide the rodent package that there is a weekly service treatment done to erradicate rodents in the facility.
