Director met with a water testing analyst consultant on 6/22/2023 to discuss the second test failure, they will be replacing piping underneath the sink. Director stated they are flushing three minutes per day for three weeks per request of water testing analyst consultant. Director mention the sink is used for cleaning purposes.

On 06/09/2023 licensee failed to maintain a lead value at or below the Action Level for water lead testing resulting with values of 5.5 ppb or greater for Site ID F. Water testing results identified with Action Level Exceedance as defined in WD section 101700.3. are not deemed safe to drink.”

Exit interview conducted and report was reviewed with Assistant Principal Allison Spillman. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Appeal Rights discussed. Assistant Principal was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. |
