The investigation consisted of review of 10 children's files, interview with 3 out of 5 parents, 5 out of 5 staff indicated that no parent has advised them that they were not given the parents rights. and Daniele Dizdul, Office Adm & Rebekah Bay, Director.
The following was found: In 10 out of 10 files reviewed parent's rights were found, signed and dated. Interview with 3 out of 5 parents indicated they had received their parents rights. Interview with staff both Director and Office Adm. stated the registration packet includes the parents rights. LPA was unable to corroborate allegation.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
An exit interview was conducted with director Bay. Appeal Rights were explained. The Director was provided a copy of appeal rights (LIC 9058) 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the Regional Manager to the address listed. |
