25 | On 10/23/2025 at 8:20am, Licensing Program Analyst (LPA) Sarah Garcia conducted a case management- incident visit in response to a self-reported unusual incident report (LIC 624) received on 08/14/25 regarding allegedly lack of supervision leading to an inappropriate child on child interaction. This is a continuation of the investigation initiated on 9/18/2025. LPA met with director, Aysegul Inceoglu. A walk through of the facility was conducted, and census was taken. Total census was 57 preschool children and 9 staff.
On 08/14/2025, a self-reported Unusual Incident Report (UIR) was filed with the Licensing Office. The facility reported that on 08/07/25 during after school care between 3-5pm, Child 1 (C1) disclosed to Parent 1 (P1) that the C1 took off clothes with another child in the classroom and teachers in the classroom (Staff #1 and Staff #2) were not allowed to see.
Throughout the investigation, LPA Garcia interviewed 3 staff, authorized representatives, and attempted to interview C1, and obtained C1's emergency information (LIC 701), and other relevant documentation.
During today's investigation, LPA interviewed 4 children, 2 staff, and obtained Child 2 (C1), Child 3 (C2), Child 4 (C3) emergency forms, one on one staff meeting agenda, and children's roster.
All staff interviewed stated they have not witnessed any recent child on child interaction. All staff stated children are supervised at all times. All children interviewed made no disclosures regarding the allegation. Authorized representatives provided feedback regarding enrollment experience but it was not enough evidence to corroborate allegation.
