32 | During LPA’s initial visit on, LPA interviewed Acting Director Kristen Schouw and Preschool Board Chair Jan Flores who joined later during LPA’s visit. Both Kristen and Jan explained to LPA the usual protocol for children’s injuries while in care at the facility. Kristen explained the order of events during the incident. The fall was observed by both staff watching the children playing in the sandbox, who were also a couple of feet away. Kristen was able to immediately tend to the child and pick them up after they fell. Kristen checked for visible signs of injury. Observed at the time were surface scratches to the right cheekbone but no bleeding. Kristen then observed a small bump begin to form on the forehead and instructed a staff member to bring them an ice pack as she retrieved the “Ouch Report” book. Kristen escorted C1 to the office to contact their parents.
During LPA’s interviews with 3 staff members, all 3 staff stated parents are contacted regarding a head injury. All 3 state for all other injuries, the level of care is decided after assessing each situation. All 3 staff were able to provide playground injury procedures to LPA.
LPA attempted to interview 4 parents, but 2 parents were unavailable and did not return LPA’s call. 2 out of 2 parents provided positive feedback regarding the facility staff. Parents did not disclose any pertinent information related to the complaint allegation.
LPA reviewed staff personnel handbook to confirm facility protocols were followed. LPA also reviewed photo and hospital discharge forms regarding C1’s injuries.
Based on LPA’s interviews conducted with Kristen Schouw, 3 staff members and review of personnel handbook, the allegation staff did not seek medical attention to daycare child may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
Exit interview was conducted with Director Mindy Weiss. The Notice of Site Visit was posted. Facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalty of $100. Director was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights.
