Licensing Program Analyst (LPA) Jenifer Tirre made unannounced visit to deliver findings on an investigation completed by the Department. LPA Tirre discussed complaint findings with Licensee Vernell Laurence and the following was determined:
During the course of investigation, the Department interviewed Staff & Clients, and made observations during department visits. Department received Client Roster, Client Individual Program Plans and medical documents. The investigation conducted revealed the following:

On June 16, 2021, the department received a complaint alleging Facility staff are not assisting residents with hygiene, Facility smells of urine and Facility is not kept at a comfortable temperature for residents.
Regarding allegation Facility staff are not assisting residents with hygiene, It was reported that Clients had poor hygiene and staff weren’t assisting clients with hygiene needs. Per interviews conducted with clients, at the time of received complaint facility had six clients in care. Six of six clients stated they take showers. CONTINUED ON 9099C

Two of six clients stated they take showers every day while four of six clients stated they take showers every other day to three times a week. Client 1, stated that Staff help them wash their hair inside sink when requested by client.

Per interviews conducted with staff, three of three staff stated that clients shower weekly and help assist with queuing clients about cleaning their body and sometimes wash hair of clients. Staff stated that one client chooses to wash up not as often as other clients.

Per observations, LPA did not observe any clients to be uncleanly or wearing dirty clothes. LPA did not observe a body odor from clients. Per observations, LPA observed each client had Hygienic supplies such as shampoo, conditioner, toothpaste and deodorant in their rooms. LPA observed rooms were neat with five of six beds made. LPA observed one bed that had sheets undone with clothes on top of bed.

Regarding allegation Facility smells of urine, it was reported that facility had a strong urine smell. Per Staff interviews, three of three staff stated that client 1 had prostate issues where they were frequently urinating. Staff interviews revealed that C1 has been taking medication and is using catheter 4 times a day to help.

Per observations, LPA did not observe a strong urine odor upon entry of facility. LPA observed a faint urine odor inside upstairs restroom where LPA observed used catheters inside restroom trash can.

Per record review, C1’s IPP states C1 has enlarged prostate and see’s urologist.

Regarding allegation, Facility is not kept at a comfortable temperature for residents, Per client interviews, clients stated they didn’t have issues with temperature and stated that facility has fans available to use inside their rooms. Per interviews with Staff, facility has provided fans and has air conditioning unit available for cooling home. Staff stated that fans are off during the day when clients are at day program, however staff encourage clients to open their windows for air to circulate. Staff state clients use fans when home.

Per observations, LPA measured temperature downstairs and measured 79 degrees. LPA measured temperature upstairs and temperature was 94 degrees with windows closed and sun hitting rooms.

Based on information gathered from investigation, the allegations Facility staff are not assisting residents with hygiene, Facility smells of urine and Facility is not kept at a comfortable temperature for residents was deemed UNSUBSTANTIATED meaning that although the allegation may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred as reported.

An exit interview was conducted with Licensee Laurence and copy of report was discussed and provided to Licensee.
