25 | Licensing Program Analyst (LPA) Giselle Lucero conducted an unannounced case management incident inspection in response to a self-report Unusual Incident dated 9/19/2023. LPA met with Director Nichole Ontiveros. LPA observed 27 preschool age children with 6 preschool age staff.
A review of staff criminal clearance records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
On 09/20/2023 a self reported Unusual Incident Report (UIR) was filed with the Licensing Office. The facility reported a child being left unattended without any supervision from staff on the playground. Director stated at 8:44 AM, children were transitioning from the playground back inside to the classroom. Staff 2 (S2) lead the children inside the classroom and Staff 3 (S3) was standing at the door to ensure the rest of the children entered. Staff 1 (S1) was instructed to bring in the last child (C1) on the playground. C1 refused to go with S1. After numerous tries, S1 proceeded to enter the classroom without C1 and let the door close behind them. A grandparent arrived to drop off a child and knocked on the door, letting the staff know there was a child on the playground. S1 assumed there were other teachers outside. District Manager reviewed video surveillance and discovered the door was closed for approximately 44 seconds. Director stated S1 has been terminated, effective 09/20/2023.
During today’s visit, LPA obtained an updated copy of the children’s roster, interviewed 3 staff and requested video footage from district manager. Due to insufficient information available at this time, the reported incident needs further investigation.
Exit interview was conducted. The Notice of Site Visit was posted. Director Nichole Ontiveros was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.
