25 | Licensing Program Analysts (LPA) Giselle Lucero conducted an unannounced case management investigation regarding an observation which occurred on 12/18/2024. LPA observed 15 infants and 3 staff during nap time.
A review of facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
On 12/18/2024, LPA Lucero observed the facility made alterations to one of the infant rooms. The infant room previously had 1 out of the 4 walls as a half wall, the wall has now been altered to a full wall. The infant room is now a fully enclosed classroom. Director stated alterations began on 11/12/2024 and was completed on 12/15/2024. LPA did not observe the facility to have reported the alterations to the licensing office.
Based on LPAs' observation, the following deficiency was observed in accordance to California Code and Regulations, Tittle 22, Division 12, Title 22, Chapter 1 101237 (a) Alterations to Existing Buildings or New Facilities 622. A technical violation was given, please see attached LIC9102.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Nichole Ontiveros.
