Licensing Program Analyst (LPA) Patricia Duron conducted a Case Management Inspection to investigate an Unusual Incident Report (UIR) which was self-reported to our office on 10/20/2021. Upon entry to the facility, LPA Duron met with authorized designated representative, Lead Teacher, Melanie Myers. LPA observed 0 school children in care with 3 staff members providing care and supervision. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

The Self-reported incident received on 10/20/2021 stated child 1 (C1) left the Facility without informing facility staff. Based on information that was obtained the facility was unaware of C1’s whereabouts for about 50 minutes. LPA Duron conducted this inspection to review facility’s procedures to ensure facility is providing care and supervision to children in care. LPA requested, the Facility Roster (LIC 9040). LPA observed Name to face log, sign in and out sheets, and entrance/exit doors. Facility uses The Hubbe app where parents sign child in/out, as well as staff. The Hubbe indicates and totals how many children are present, how many children have been signed out, and how many children are absent. An interview with Lead Teacher, Melanie Myerf, was conducted. Melanie Myers stated name to face log is updated every 30 minutes as staff confirm total of number of children in attendance. Myers stated staff communicate verbally using walkie talkies informing one another which child has been picked up and signed out.

Lead Teacher, Myers stated staff members take a required Health and Safety Training. Myers stated, staff will contact child’s parents/authorized representatives if child does not arrive within 4 to 5 minutes after school has let out. Staff will contact elementary front desk to confirm if child attended or is absent for the day. Child's parents/authorized representatives will inform staff if child is enrolled in an after school program, activities or sports. Staff will not allow children to attend activities unless child’s parent/authorized representatives has confirmed child is able to attend.

In the areas that were evaluated, NO deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit.

The Notice of Site Visit was posted. Lead Teacher was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The Lead Teacher was provided a copy of their Appeal Rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. Site Supervisor was informed all appeals must be in writing and received by the Licensing office within 15 business days.
