25 | Licensing Program Analyst (LPA) Patricia Duron conducted a Case Management Inspection to investigate an Unusual Incident Report (UIR) which was self-reported to our office on 10/20/2021. Upon entry to the facility, LPA Duron met with Center Manager, Marsha Linguist. LPA observed 34 school children in care with 5 staff members providing care and supervision. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
The Self-reported incident received on 10/20/2021 stated child 1 (C1) left the facility without informing facility staff. Based on information that was obtained the facility was unaware of C1’s whereabouts for about 50 minutes. LPA Duron conducted this inspection to review facility’s procedures to ensure facility is providing care and supervision to children in care. LPA requested, the Facility Roster (LIC 9040). LPA observed Name to face log, sign in and out sheets, and entrance/exit doors. Facility uses The Hubbe app where parents sign child in/out, as well as staff. The Hubbe indicates and totals how many children are present, how many children have been signed out, and how many children are absent. An interview with Center Manager, Marsha Linguist was conducted. Linguist stated name to face log is updated every 30 minutes as staff confirm total of number of children in attendance. LPA reviewed facility's name to face logs. Linguist stated staff communicate verbally using walkie talkies informing one another which child has been picked up and signed out.
