The LPA registered the licensee to receive PINs during the meeting.
The licensee agreed to do the following to bring the facility into compliance no later than the following dates:
The licensee stated that she has corrected all the since LPA’s visit. The licensee removed all loose articles from both cribs while the LPA was present. The licensee received a copy of safe sleep regulations (Spanish version) and a "What a safe sleep environment looks like” brochure (Spanish version). The licensee reviewed the regulations and sent a written declaration via email to the LPA on 10/31/23 stating the corrections had been completed.
The licensee will develop a protocol for the procedures listed below and email it to the LPA by 4/27/24:
1. Completing infant safe sleep. This includes conducting and logging the 15-minute checks, keeping play yards/cribs free of loose articles, placing infants to sleep in an appropriate environment as required per regulations, and who will be primarily responsible for ensuring the facility is up to regulation.
2.Train staff on who is Community Care Licensing and its role with licensed home.
During the meeting, the licensee was given the opportunity to discuss the deficiencies and plan of action above.
The licensee stated that C1 has been accustomed to sleep in the same area as the other children. The licensee stated that all children, including the infants, sleep in the same area of the day care where the licensee has a visual while they nap. The licensee stated she modified the way she observes safe sleep regulations and other licensing regulations. The licensee stated the parents received a copy of the LIC9224.
LPA A. Silva informed licensee Rosalba Miranda that this licensing report dated 4/10/24 documents an Informal Visit at the Regional Office. Rosalba Miranda that a copy of this licensing report must be provided to parents or guardians of all clients currently enrolled by the next business day or by the next day the children are in care; a copy of this report must be provided to the parents or guardians of all newly enrolled clients for 12 months from the date of this report.
A signed Acknowledgement of Receipt of Licensing Report (LIC 9224) form, or another written equivalent statement, must be placed in the child's file for verification of receipt of the report.
