On 3/5/2026, at 2:20pm Licensing Program Analyst (LPA) Anna Chan conducted an unannounced Complaint investigation inspection. This is to deliver the findings of the investigation initiated on 12/30/2025. Upon arrival, the LPA met Director Iris Antuna and informed them of the purpose of visit. Census was taken. LPA observed 28 infant children and 8 staff.

The Department received a complaint on 12/22/25 alleging Staff did not ensure that the facility was kept free of rodents

During the course of investigation, LPA interviewed 7 staff, 4 parents, and reviewed facility files. No children were interviewed due to children are non-verbal

Reporting Party (RP) stated there is rat infestation in the facility.

LPA interviewed 7 staff. 5 of 7 staff stated they are aware of rodent infestation and have seen droppings in the infant yard. Staff 1 (S1) stated they have seen rodents near the bushes on top of the infant yard and it is an ongoing issue. S1 stated they have seen droppings outdoors and there was sticky mouse traps and box traps close to the AC unit in the play area. S1 stated children have no access to droppings and they clean the area before children play in the yard. Staff 2 (S2) stated there was a rodent issue in the Summer of 2025 and have seen a little mouse in the infant yard. S2 stated they clean the area before going out to make sure children are safe. Staff 3 (S3) stated they have not seen rodents but are aware of the issue from a year ago. Staff 4 (S4) stated they have seen rodents in the classroom on or about December 19, 2025, and killed the rodent. S4 stated the rodent was under a mat and S4 used a paper towel to catch it. Staff 5 (S5) stated they have not seen rodents or droppings at the facility. Assistant Director stated there were rodent sightings, and they have placed traps outside not accessible to children and there are 2 pest control companies who do the maintenance. Assistant director also stated it is an ongoing issue, and they are trying to resolve it.

Director stated the facility will request for a rodent focused service to resolve the issue.

On 12/30/25 LPA observed in the outdoor play area, there was an AC unit barricaded by a wired fence, there is a black box placed on the ground, and the assistant director stated it is mouse trap. Next to the mouse trap are rat droppings. Children have no access in the area.

During record review, the facility showed proof of Pest Control Invoice with service date 10/16/25 for Rodent Proofing “Closing exterior entry points for rodents”.

LPA interviewed parents, none of the parents interviewed disclosed information that supported the allegation.

Based on observation, interview and record review, the preponderance of evidence standard has been met, therefore the above allegation that Staff did not ensure that the facility was kept free of rodents is found to be SUBSTANTIATED. See LIC9099D for 1 Type B violation.


Exit interview was conducted with Director Iris Antuna. Report and deficiency were discussed. The Notice of Site Visit was posted and must remain posted for 30 consecutive days. Appeal Rights were provided.

A review of the Facility Personnel Report Summary conducted on today’s date indicates all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions.

The Department received a complaint on 12/22/25 alleging Staff did not maintain proper staff-infant ratios.

During the investigation, LPA interviewed 7 staff members, and 4 parents, obtained children’s (infant) and staff attendance for the month of December 2025, Child Supervision Report (CSR)

During staff interview, 6 of 7 staff stated they have not witnessed a class being out of ratio. Staff stated they will send a group message if additional help is needed in the classroom. Staff 5 (S5) stated they help cover the classroom to be in ratio.

LPA reviewed CSR for the month of December 2025. CSR showed the facility is in Ratio.

LPA interviewed 4 parents and none of the parents interviewed disclosed any information that could support the allegation.

NO children interviewed due to children being non-verbal.

Based on observation, interviews conducted and records reviewed, although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged Staff did not maintain proper staff-infant ratios did or did not occur, therefore the allegation is UNSUBSTANTIATED.
