On 09/01/2026, at 1:30pm Licensing Program Analyst (LPA), Anna Chan conducted an onsite inspection for the purpose of an unannounced Annual Random Inspection. LPA met with Director Iris Antuna and toured the facility inside and outside and the floor and yard plan (LIC 999) were verified. Children start arriving at 2:40pm and another group arrived at 3:08pm. Census was taken. The overall census observed was 10 school-age Children and 1 staff.

During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 6:30a.m.- 6:30p.m., Monday through Friday. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Kindercare has a Master Roster which is located on License# 304270926 and 304270924 KINDERCARE LEARNING CENTER 855 PASEO WESTPARK, IRVINE 92604.

During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. PM snacks is provided by the facility. Food is prepared on site. Food prep areas were clean and sanitary. Food is properly stored. Menus were posted where they could be reviewed by parents.

Floors, equipment, and furniture were clean and were observed. There is drinking water available to children indoors by Brita water dispenser and children have their own sports bottles properly labeled with their names, the children's bathrooms are clean and sanitary. The facility has a working carbon monoxide detector and fire extinguisher. The facility has conducted an emergency drill within the past six months on July 16, 2026. Facility met all posting requirements.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The outdoor activity space was inspected. The playground was enclosed by a fence at least four feet in height. The cushioning material in the playground is pour-in place rubberized material around the climbing structure such as slide and other climbing equipment which appears to be thinning. Director stated they will discuss the surface area with their District Leader. There is also a grassy area around the playground. Drinking water in the outdoor activity space is provided by Brita water pitcher and refillable sports bottles with the child’s name on it. There are no bodies of water present at the facility, and none was observed during today’s visit.

5 Children's records were reviewed, and there was a separate, complete and current record for each child. Electronic sign in/out procedure was reviewed through Care Connect.

The facility does transport children to and from school. The facility starts the van run around 7:15am and in the afternoon, van run starts picking up at 1pm.

1 Staff file was reviewed during the facility inspection on this date. Health screening and immunization as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1596.7995 (a)(1) a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles.
Proof of immunization against pertussis, measles for the facility were reviewed and within compliance.

Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporting training, and to renew the training every two years. At least one staff member possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 11/2027.

Currently, there are no medications in the facility. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018 requires all licensed Child Care Centers (CCC’s) constructed before January 1, 2010 to test their drinking water for lead contamination between January 1, 2020 and January 1, 2023, and then every five years after the date of the first test. Community Care Licensing was notified that lead water testing conducted at the facility on 12/03/2022 failed allowable limit for lead. The purpose of today’s visit is to follow up lead testing results of Action Level Exceedance (ALE) on yard fountains(bubblers) Outlet R (ALE 47) per report provided to the Department . However, Lic 999 lists outlet as S and the lab tests from facility also list outlet as S as being the outlet that measured ALE of 47. Director stated they use Brita water pitcher and does not use water fountains for drinking. Kitchen faucet that failed the test were not used for food prep.

Instructions for required lead testing are outlined in PIN 21-21.1-CCP (Supersedes PIN 21-21-CCP) This PIN which contains Written Directives, have the same force and effects as the regulations contained in Title 22 of the California Code of Regulations. Lead Sampling Testing conducted on 12/03/2022.

In the areas that were evaluated, no deficiencies were observed in the California Code of Regulations Title 22, Division 12 at the time of visit.

Exit interview conducted and report was reviewed and discussed. with the Director Iris Antuna. A notice of site visit was given and must remain posted for 30 days. Appeal Rights were provided.
