25 | Licensing Program Analysts (LPA) P Rivas and A Silva conducted an unannounced case management visit to investigate an incident of 12/09/22. LPAs were met and assisted by Director Amanda Bartlett. Rooms # 3 and 4 were toured. Room 3 had 12 napping children and one teacher. Room 4 had 15 napping children with one teacher and one aide. Facility is operating within ratio.
A review of staff records on this date indicated that all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions.
Regional office was notified that the facility closed unexpectedly on 12/09/22 and a few other dates but only notified parents the day of early closure.
Interview with Director Amanda Bartlett stated that she can recall only having to close early once in December 2022. States the closure was due to staffing . A closing teacher was not able to attend work due to personal reasons and facility closed at 4:00pm. Normally closed 6:30PM. Director advised that in the enrollment agreement there is a section for emergency situations titled :Closures due to weather, emergency or illness. "...If the center has to close early, you will need to arrange for your child's pickup. If you need information about an emergency affecting your center and are unable to reach the center, please contact your District Leader of Family Support team. There's no tuition reduction for holidays or for center closures. "
Based on the information available The facility has shown it is operating within it's enrollment agreement.
No deficiencies were cited during todays evaluation.
Exit interview was conducted with Director Amanda Bartlett . Notice of Site Visit was posted during the visit. Director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Director was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First level appeals should be sent to the regional manager to the address listed above |
