25 | Licensing Program Analyst (LPA) Nunez conducted an unannounced case management incident inspection in response to a self-report Unusual Incident dated 2/7/2024. LPA met with Assistant Director Daisy Garcia. Director Amanda Barlett was not present today due to being off. LPA observed 23 preschool age children and 2 staff members. A review of staff criminal clearance records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
On 2/7/2024 a self-reported Unusual Incident Report (UIR) was filed with the Licensing Office. The facility reported that on 2/7/24, a family member called the childcare facility to inform them that abrasion was observed on child#1 (C1) by C1’s family member on 2/7/24 when changing C1’s diaper, after leaving the daycare facility. During today’s visit, LPA obtained a copy of the children’s and staff roster, reviewed C1's file, obtained C1’s report that contained C1’s meal and diaper change charts and sign in/out sheet for the day of 2/7/24, and interviewed 2 staff members. Due to insufficient information available at this time, the reported incident needs further investigation.
Exit interview conducted with Assistant Director Daisy Garcia and notice of site visit must be posted at entrance of facility. Notice of site visit is to remain posted for no less than 30 days. Failure to post will result in civil penalties of $100. Appeal Rights was explained.
