This is a follow up UIR Case Management Inspection from visit dated 02/12/24. A case management inspection was conducted today by Licensing Program Analyst (LPA), Nunez who met with the Director, Amanda Barlett. A self-report incident by the facility was received by regional office on 02/07/24 which stated on 2/7/24, a family member called the childcare facility to inform them that abrasion was observed on child#1 (C1) by C1’s family member when changing C1’s diaper, after leaving the daycare facility.

Census was taken today and there was a total of 30 preschool age children with a total of 3 staff. Census was taken during nap time. A review of criminal record clearances indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

During the investigation, LPA interviewed 4 staff members and obtained a copy of the roster, sign in and sign out sheet and diaper changing chart for 02/7/24, a doctor’s note, and reviewed C1’s file. During interviews, Staff#1 (S1) disclosed that on 2/7/24 S1 was assigned to be in the classroom where C1 was located. S1 stated that S1 is usually not in that classroom. S1 stated that on that day S1 was alone with 12 children. S1 stated S1 changed C1’s diaper in the morning only and it was only a wet diaper. S1 stated S1 did not observe anything on C1’s private part while S1 changed C1’s diaper. S1 stated during the day when C1 was in the classroom S1 did not see C1 being uncomfortable. Staff#2 (S2) disclosed that on 2/7/24 S2 was in and out of the classroom due to S1 being alone with 12 children. S2 stated that S2 did not change C1’s diaper on that day. S2 stated when S2 came in the classroom S2 was helping clean up the toys from the floor and C1 was helping S2 clean up the toys. S2 stated C1 seemed fine and didn’t seem hurt during the time C1 was helping S2 pick up the toys. S2 stated S1 changed C1’s diaper on 8:30am and C1’s next diaper change was going to be around 12:00 pm but C1 was already picked up from the facility. On the diaper chart that LPA obtained it stated that C1’s diaper change was at 8:24am and it was a wet diaper. On the sign out sheet it was logged that C1 was sign out from the facility at 10:20am.

Based on LPA interviews conducted, and records/documentation reviewed, there is not enough evidence to support any violation of Title 22 regulations. No Title 22 deficiencies were cited during today's inspection.

Exit interview was conducted. Notice of Site Visit was posted during the visit. Director Amanda Barlett was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Appeal rights provided and explained. The Director was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First level appeals should be sent to the regional manager to the address listed above.
