Licensing Program Analyst (LPA) Dean Thompson conducted an unannounced case management incident inspection in response to a self reported Unusual Incident dated 10/29/2024. LPA met with Director Jasmin Canales and toured the facility. LPA observed 56 preschool children with six preschool staff.

A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During the visit, it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios.

On 10/29/2024, a self reported Unusual Incident Report (UIR) was filed with the Licensing Office. The facility reported a child being left unattended without any supervision inside classroom #5 dated 10/28/2024.

Director reported a parent arrived to the facility to pick up their child (C1) and walked into classroom #5. While inside the classroom, parent observed C1 was left alone without any staff present inside the bathroom of classroom #5.

Director stated the roster was checked and the roster showed at 5:20 PM, classroom #5 transitioned across the hall into classroom #8. Staff #1 (S1) was in charge of classroom #5 at the time of the incident and had marked all children as transitioned to classroom #8 but didn't realize C1 was left behind. Director stated C1 was left alone without any supervision for four minutes.

Records reviewed indicated S1 was supervising 10 children inside classroom #5 during the time of the incident and before transitioning the children to classroom #8.

Based on records reviewed, Directors statements, and the self reported incident, it was revealed that child (C1) was left alone on 10/28/2024 without any adult supervision inside classroom #5 for four minutes. The following violation Responsibility for providing care and supervision was revealed today and is being cited in accordance with California Code of Regulations, Title 22, Division 12, Chapter 1, 101229 (a)(1) . Please see the attached LIC 809D page.

A previous licensing report was issued on 8/6/2024 giving notice of the same violation. Because the facility been cited for repeating the same violation within 12 months, a civil penalty has been assessed.

LPA Thompson informed Director Jasmin Canales that this report dated 11/5/2024, document(s) one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Thompson informed Director Jasmin Canales to provide a copy of this licensing report dated 11/5/2024, that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview conducted and report was reviewed with Director Jasmin Canales. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.



Appeal Rights and deficiencies were discussed. The Director was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.

Director stated they have provided extra staff to support staff #1 during transition periods. Director also stated classroom #5 has been given a closing teacher so that the classroom no longer has to transition to other classrooms.
