25 | Licensing Program Analyst (LPA) Carmen Odom and Alma Castro conducted an unannounced case management incident inspection in response to a self-report Unusual Incident dated 2/14/2024. LPA met with Assistant Director Sheryl Nishinaka. LPA observed 43 preschool age children and 5 staff members, children were playing. A review of staff criminal clearance records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
On 2/14/2024 a self-reported Unusual Incident Report (UIR) was filed with the Licensing Office. The facility reported that on 2/14/24, Parent #1 (P1) spoke with Staff #2 (S2) regarding what Child #1 (C1) disclosed the night before. C1 told P1 that Staff #1 (S1) pushed C1 into the restroom while other children were waiting in line.
During today’s visit, LPA obtained Staff #1 (S1) contact information. S1 and C1 were not at the facility during today's visit. Due to insufficient information available at this time, the reported incident needs further investigation.
Exit interview was conducted with Assistant Director Sheryl Nishinaka. The Notice of Site Visit was posted. Director was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. |
