25 | Licensing Program Analyst (LPA) Romy Castanon conducted a case management inspection to follow up on a self-reported unusual incident that was submitted to the Orange County Regional Licensing Office on 03/01/2023. It was reported by Director Lynn Porter via telephone.
A tour of the facility’s Room 5 was conducted. There was a total of 11 children and 2 staff members. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances.
The unusual incident report written by the Director stated that at the end of the day the Staff #1 (S1) combined their children in another classroom. Child #1 (C1) began playing rough with the other children, including a sibling. Staff were unable to deescalate the child and chose to send them to the office. The child ran toward the door. S1 intercepted the child but the child punched S1 in the face. Director suspended the child for one day. The sibling became upset because C1 stayed home and began misbehaving. Parent became irate and refused to sign the behavior report. Parent made claims that C1 alleged inappropriate behavior by S1. Director and District Leader placed S1 on administrative leave effective immediately pending internal investigation outcome.
During today's visit, LPA spoke with Director for further detailed information not found on UIR. The child was on a behavioral contract for aggressive behaviors toward peers and staff. Contract detailed that further incidents would require suspension and a behavioral specialist to assist them. During record review, parent signed all daily behavioral reports, as well as, the behavioral contract dated 02/08/2023. Director and District Leader concluded that S1 was not left alone with C1 after reviewing the facility Child Supervision Records. S1 returned to work on 03/02/2023. Parent has since reported the alleged incident to the Santa Ana Police Department At the time of writing, the police department investigation has not concluded.
