32 | On 3/19/25 the Orange County Child Care Office received a complaint alleging (1) Staff left daycare children unattended while in care and (2) Staff did not properly report incidents involving daycare children. Reporting Party (RP) stated the following: More than one instant a child had been left on their own without an adult being present. The first incident occurred on 2/24/25, when Staff#1 (S1) forgot Child#1 (C1) out on the playground and left them out there for a while. Several minutes later, the classroom went outside to the playground, and Staff#2 (S2) and Staff#3 (S3) noticed C1 alone without a staff member being present. The second incident occurred on 2/28/25. Staff#6(S6) had been walking down the hallway, and Child#2 (C2) from S1’s classroom opened the door. Upon going inside, S6 noticed C2 was alone in the classroom. C2’s classroom was outside in the playground, and they had forgotten C2. On 4/17/25, Child#1 was left behind unattended by S1 who had previous incidents with leaving children in the classroom and outside.
During the investigation, LPA toured the facility, obtained a copy of the children’s rosters, all-staff meeting agenda from 3/14/2025, counseling reports, and the personnel report. LPA also conducted interviews with 13 staff members, attempted to interview 6 children but only 3 children qualified for an interview, and 3 parent interviews.
Regarding allegation: Staff left daycare children unattended while in care
During staff interviews, 3 staff members that were outside in the playground confirmed that C1 was alone in the middle of the playground on 2/24/25. S2 stated that C1 was by himself, unsupervised in the middle playground. S3 stated that while S3 in the larger playground trying to line up one of the children, S3 looked up and noticed C1, who was alone in the middle playground without any supervision. Staff#4(S4) also confirmed that S4 was outside during the incident and saw C1 alone in the middle of the playground.
S6 stated the following: On 2/28/25, S6 had walked around the hallway and S6 had 2 children with S6, the moment S6 walked S6 noticed somebody opening a classroom door very slowly and it was C2. S6 asked C2 if C2 had been alone, C2 did not respond to S6 but S6 noticed that nobody was inside the classroom. S6 took C2 to the director’s office. The director and assistant director stated the following: Staff#7 (S7) and Staff#8 (S8) they were in transition to the playground, and they left C2 in the classroom. S7 and S8 confirmed that they left C2 inside the classroom alone.
