32 | During the investigation, LPA interviewed 7 staff members and obtained a copy of the staff timecards, children’s sign in & out sheets, and documents pertinent to the investigation. LPA reviewed staff communication on 06/30/2025 at 9:33 a.m. Staff #1 (S1) reported having 12 children and requested support, noting that two more children were expected. At 9:40 a.m., S1 stated S1 had 13 children and again requested support. At 9:43 a.m., another staff member responded that they would arrive shortly. Staff #6 (S6) reported that on July 1, 2025, S6 had 10 children and notified the front office that they were approaching 12. S6 later informed the office that they had 13 children and needed support. S6 stated that an additional staff member arrived approximately 10 minutes after the call.
During interviews, 3 of 7 staff members confirmed that the facility has operated over capacity. 2 staff members confirmed it was on 06/30/2025 and 07/01/2025.
On 8/21/25, LPA was able to interview 3 parents, and the 3 parents did not disclose any concerns regarding the allegation.
Based on information gathered from LPA’s 7 staff interviews, record review, and 3 parent interviews, the preponderance of evidence has been met; therefore, the allegation of Staff did not ensure proper teacher-child ratios were maintained at the facility is SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter 1, Section 101216.3 Teacher-Child Ratio is being cited on the attached LIC9099D. Please refer to attached 9099D for documentation of deficiencies.
LPA informed the Director that this report cites a Type A violation and shall be provided to parents/guardians of children currently enrolled and to parents/guardians of children newly enrolled at the facility during the next 12 months. Parents/guardians must sign Form LIC9224 and needs to be kept in each child's file.
Exit interview was conducted with Director, Katherine Yeakel. Notice of Site Visit was posted during the visit. The facility representative was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.
Appeal Rights were explained. The Director was provided with a copy of the appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First-level appeals should be sent to the regional manager at the address listed above.
