Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management inspection of the facility for the purpose of the facility moving to a different building on this large campus by decreasing their capacity from 194 preschool children to 140 preschool children and adding 12 toddlers from Toddler Option Component to their preschool program. The facility has been licensed for 194 children in rooms 102, 103, 104, 105, 106, 107, 108, and 109. LPA met with director, Jennifer Eggers who gave LPA a tour of the facility. The facility is located on the ground of Mariner's Church. There were 47 preschool children with 16 staff present today.

The facility has requested to provide care and supervision to 140 preschool children ages 2 to 5 years old and 12 toddlers ages 18 months to 36 months old, Monday to Friday from 7:00 am to 6:00 pm by moving to a new building on this existing campus (Building 7 in upper campus).
Parents sign in and out their children electronically by entering the lobby of this building and a staff person will escort the child to the classrooms behind a double door entrance.
A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
The facility provides morning and afternoon snacks and parents provide lunch to children. The children are dropped off and are singed in and out at the entrance electronically.

All areas identified on the Facility Sketch were inspected. If children become ill during the day, they will rest in the director's office, and use the adult restroom until sick children are picked up.
The new rooms in the new building were measured. There are a total of 8 rooms designated for preschool children and 2 assembly rooms for additional activities on occasions.

Following was observed in all the rooms:
Age appropriate furniture and activities including educational, arts & crafts items.
Rooms are well ventilated with proper lighting and shade.
According to director the smoke detector and Carbon Monoxide are combined. LPA observed the device installed in the ceiling of each room.

The measurement of the rooms according to updated numbers are as below:
Room 121 is: 864 square feet
Room 122 is: 986 square feet
Room 123 is: 704 square feet
Room 126 is: 1739 square feet (Assembly room 1)
Room 129 is: 955.50 square feet
Room 135 is: 1053 square feet
Room 137 is: 1288 square feet (Assembly room 2)
Room 143 is: 1001.70 square feet divided by 35 square feet accommodates 28.62 (29 children). This room is designated for 12 toddler option children ages 18 moths to 36 months old. The Toddler Option Room has 2 toilets and 2 sinks inside a restroom in the classroom.
Room 144 is: 1173 square feet
Room 147 is: 786.80 square feet
Total indoor measurement for the rooms are 10551 square feet divided by 35 square feet = 301.46
(301 children)

Toilets: 19 X 15= 285 children
Sinks: 27 X 15 = 405 children.
Urinals: 3 x 15 = 45 children
There are sinks and toilets inside the classrooms and in the hallway.

Outdoor Activity Space:
Total measurement of the outdoor space is: 6733.4 square feet divided by 75 square feet=89.77
(90 children).
Playground is completely enclosed. Outdoor activity area is supplied with age and size appropriate equipment with adequate of cushioning materials such as turf and sand under climbing structure and slide. Shade. Shade is provided by trees, awning, and building structure.

LPA reviewed with director the infant sleep log for every 15 minutes check on children 24 months old since this program has toddler option component and children under 24 months old shall be checked every 15 minuets during nap time. LPA gave a copy of the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. The director's Immunization, Mandated Reporter Training, Health and Safety Training are current. Her CPR and 1st aid is EMSA approved and expires 7/29/2025.

The director, Jennifer Eggers was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care- licensing/subscribe and select the Child Care option to receive email communication. Exit interview conducted and report was reviewed with the director, Jennifer Eggers. Fire clearance approval has been received in our office to approve the capacity of 140 preschool children and 12 toddlers.

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

Based on today’s measurements, the sinks and toilets availability, the request of the facility for moving to building 7 and decreasing their capacity to 140 preschool children and add 12 toddlers from toddler option components in rooms 121, 122, 123, 126, 129, 135, 137, 143, 144, and 147 will be final reviewed and discussed with management in the Regional Office and after receiving the following corrections. The facility will be notified if any additional information is required. Please send the following corrections within 30 days of today's date.


1- Final picture of all the classrooms after final set up since the facility has not moved in completely.
2- Securing wooden cozy area in rooms 123, 129, and 144.
3- Removing low level mirror on the wall in rooms 144. The edges of the mirrors are sharp.
4- Please make sure all the tall shelves of the hazard to tip over easily are secured in all rooms.
5- A photo of after repairing the lower rusted fence area on the playground.
6- Photo after posting all the posting requirements in public area. The items for posting was given to director
7- Final photo of the preschool playground after expanding by moving back the fence from infant playground to preschool playground.
8- Waiver request for sharing the playground with toddler option and requested capacity. For waiver please send waiver request letter, LIC 999, LIC 500, outdoor schedule, and photo of the playground.
