32 | On 8/21/25, LPA was able to interview 3 parents, and the 3 parents did not disclose any concerns regarding the allegatiosn.
Based on information gathered from LPA’s 11 staff interviews, 6 children's interviews, record review, and 3 parent interviews, the preponderance of evidence has been met; therefore, the allegations of (1) Unqualified staff is providing care and supervision and (2) Children were taken to an unlicensed building are SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter 1, Section 101216.1(c)(1) Teacher Qualifications and Duties and (2) 101161(a) Limitations on Capacity are being cited on the attached LIC9099D. Please refer to attached 9099D for documentation of deficiencies.
Exit interview was conducted with HR Manager, Sara Crippen. Notice of Site Visit was posted during the visit. The facility representative was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.
Appeal Rights were explained. The HR Manager, Sara Crippen, was provided with a copy of the appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First-level appeals should be sent to the regional manager at the address listed above.
