Licensing Program Analysts (LPAs) A. Nunez and D. Valdez-Santana conducted an unannounced complaint investigation. This is a continuation of the investigation initiated on 8/18/25. Upon arrival, LPA met with Human Resource (HR) Manager, Sara Crippen, and informed the HR Manager the purpose of the visit is to deliver complaint findings. Census was taken and observed with a total of 55 preschool children and 14 staff members.

A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During today’s inspection the facility was operating within its licensed capacity and within compliance of staffing ratios.

On 8/13/25 the Orange County Child Care Office received a complaint alleging (1) Unqualified staff are providing care and supervision (2) Children were taken to an unlicensed building. Reporting Party (RP) stated the following: On August 8, 2025 children were taken into a different building for soccer. The event happened in a space that was not licensed to have children. Children are being left alone with teachers that do not have all their units. Staff#7(S7), Staff#8 (S8), and Staff#9 (S9) are some of the teachers that have been left alone with the children although they don’t have all their units.

Regarding allegation: (1) Unqualified staff is providing care and supervision

During the investigation, LPA interviewed 11 staff members and reviewed 3 staff members files. S7 confirmed that S7 has been alone with 4 children for at least 30 minutes. Staff#10 (S10) also stated that S7 has been alone with the children in the playground. LPA reviewed S7 file and S7 has 6 Early Childhood Education (ECE) units. S8 stated S8 is the music teacher and helps in the classrooms when it’s needed. S8 denied the allegation. However, S10 and S9 stated that S8 provides individual music lessons for children who pay for them. The lessons were during school time and S8 would stay alone with the children to do the music lessons. LPA reviewed S8 file and S8 completed 3 ECE units and is enrolled to take 6 ECE units in Fall 2025. S9 denied the allegation; however, S10 stated that S9 has been alone with the children. S10 has left S9 alone with the children because S10 thought S9 had 12 ECE units. LPA reviewed S9 file and S9 has completed 3 ECE units.

Regarding allegation: (2) Children were taken to an unlicensed building

During the investigation, LPA interviewed 11 staff members and 6 children. 11 out of 11 staff members stated that on August 8, 2025, there was a soccer clinic for the children in the Youth building. 7 out of 11 staff members stated that they stayed at the clinic for approximately 15–20 minutes. The previous director stated she informed the parents about the clinic but did not tell the parents the location of the clinic. The Youth building is not a licensed room. 6 out of 6 children stated they attended the soccer clinic.

On 8/21/25, LPA was able to interview 3 parents, and the 3 parents did not disclose any concerns regarding the allegatiosn.

Based on information gathered from LPA’s 11 staff interviews, 6 children's interviews, record review, and 3 parent interviews, the preponderance of evidence has been met; therefore, the allegations of (1) Unqualified staff is providing care and supervision and (2) Children were taken to an unlicensed building are SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter 1, Section 101216.1(c)(1) Teacher Qualifications and Duties and (2) 101161(a) Limitations on Capacity are being cited on the attached LIC9099D. Please refer to attached 9099D for documentation of deficiencies.

Exit interview was conducted with HR Manager, Sara Crippen. Notice of Site Visit was posted during the visit. The facility representative was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.

Appeal Rights were explained. The HR Manager, Sara Crippen, was provided with a copy of the appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First-level appeals should be sent to the regional manager at the address listed above.
