Licensing Program Analyst, (LPA) P Rivas conducted an unannounced annual inspection and met with Amanda Wilson, Lead Teacher and spoke with Interim Director Jennifer Eggers over the telephone. LPA was advised that Ms. Wilson is designated to meet with the Department.

A review of staff records indicates all facility staff or individuals who require caregiver background checks have received a criminal record clearances or exemptions and a child abuse index clearance. A Facility Risk assessment was conducted to determine the level of Personal Protective Equipment (PPE) was needed for an on-site inspection.
A walk through of the facility was conducted with Ms. Wanda Kao, Registrator, and later assisted by Ms. Wilson. Hours of operation are from 7:15 am to 5:30pm, Monday - Friday. LPA observed; 2 teachers in each classroom; Rm 102- preschool Rm 103 half day program ; Rm 104- 10 , preschool ; Rm 105- 17 3 yr olds; Rm 106 -11 pre school ; Rm 107- 15 - preschool ;Rm 108-17 -pre-K children; Rm 109- 19 -prek children . Due to COVID 19 guidelines, LPA observed staff wearing face masks, and were following CDC and Dept of Public Health Guidelines.

During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Poisons/Hazardous Items are not kept on the premises. The facility does not provide lunch for children but provides snacks. Floors, equipment, and furniture were clean and observed to be in good repair and free of sharp edges. There is drinking water available to children indoors by children's own water bottles.

The staff was informed that the Criminal Record Statement (LIC 508) has been updated, and the facility must now use the new form with revised date 7/15. Director was also informed that the LIC 508 must be submitted with all Criminal Background Clearance

Health screening and immunizations as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for staff were reviewed and within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporting training, and to renew the training every two years. All staff had current mandated reporter training, Each staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 07/31/2023.

LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The outdoor activity space was inspected for compliance. The playgrounds are enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The cushioning material , sand and rubber around the climbing equipment, slides and other similar equipment appeared to be enough to absorbs falls. Drinking water in the outdoor activity space is provided by Sport bottles with the child’s name on it or cups and water jugs. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility. The facility grounds were safe, sanitary and in good repair.

Due to time constraints the annual inspection will be continued on a different date.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Ms. Wilson.
