# 2 was in the middle, and staff # 3 was in front of the line. Child # 1 (23 months old) refused to move with the group. Child ran towards the adjacent playground and insisted not going to the classroom with the group. Staff # 2 and staff # 3 with 8 toddlers reached the toddler classroom passing two gates and around the corner of the building inside the facility. Staff # 1 stayed with child # 1 on the playground while the rest of the group had gone inside the toddler room. At this time staff # 1 decided to leave child # 1 unattended until staff could go inside the classroom and get help from another staff. Therefore staff # 1 walked 100 feet away from where child # 1 was and went around the building structure, passing two gates, asking staff for help to bring child # 1 inside. Child # 1 was left unattended for 30 seconds to a minute until staff # 1 got help. The estimated time was confirmed by the witnesses.
Based on the interviews with the director, and staff # 2, and 3, reviewing the written incident dated 7/8/2024 and the Child Supervision Record (CSR), the incident was found out to be due to lack of supervision for 30 seconds to one minute.
In the areas that were evaluated, the following deficiency was observed of the California Code of Regulations, Title 22, Division 12 on this inspection. Please see next page LIC 809D for detail of deficiency.
A notice of site visit was given to director, Barbara Brown and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to post will result in Civil Penalties of $100.00.
