Reporting Party (RP) alleges (1) Child sustained unexplained injuries while in care, and (2) Staff did not report incidents to child’s authorized representative.
Per the RP, On 5/15/2024, RP observed two open wounds on child #1 (C1) right pelvic area which RP stated likely occurred when the snaps of the cloth diaper were snapped onto the skin in the pinch point area. RP also mentioned staff did not inform RP about the injury.
During the investigation, LPA reviewed pictures, interviewed staff, interviewed parents, reviewed C1 file, retrieved an incident report, diaper logs, child supervision records, and retrieved the facility roster.
On 5/21/2024, LPA conducted four staff interviews with staff who supervised C1 on 5/13/2024, 5/14/2024, and 5/15/2024. Out of the four staff interviewed, three staff stated they have changed the diaper of C1. Three staff stated they have never pinched C1 skin with the snaps on the cloth diaper and C1 hasn’t cried during diaper changing.
LPA reviewed pictures from the allege incident. LPA was unable to determined if the markings were caused from C1 skin being pinched. Pictures reviewed do not show open wounds on C1. Diaper logs reviewed do not have any comments regarding pinched skin.
RP mentioned C1 had a long scratch on the right side of the face and was not given an incident report for the week of 5/6/2024. On 5/21/2024, during staff interviews, one staff stated C1 was scratched by another child on 4/24/2024. LPA reviewed an incident report dated 4/24/2024, for C1. Nature of the incident/accident was for a scratch on the face of C1. Incident report reviewed shows a signature of parent/guardian.
On 5/29/2024, LPA conducted parent interviews. LPA contacted 15 parents and five parent were able to be interviewed via phone call. Parents interviewed did not divulge any information pertaining to the allegations and had nothing but positive feedback.
