32 | LPA interviewed 4 staff members, 4 out of 4 staff stated they have not witnessed S1 handle child in a rough manner. S1 stated, C1 was sitting with S3, C1 didn’t want to let go of S3. C1 was holding on to S3. S1 helped C1 walk to the carpet and S1 asked if S1 could help C1 up. S1 and C1 walked together to the carpet. S1 talked to P1 that day about C1 crying because S3 had to leave for the day and C1 was upset and was crying.
S2 stated, S2 has not witnessed S1 handle a child in a rough manner. S1 talks a lot to the children. S1 uses her words when working with challenging behavior children.
S3 stated, S3 has not witnessed S1 handle any child in a rough manner. S1 teaches staff how to handle children with a gentle manner. When staff members are handling a challenging child, S1 reminds staff to handle carefully and S1 roles models for staff.
LPA Duron interviewed 3 children. All 3 interviewed children did not reveal any staff
violating their rights.
LPA Duron interviewed 4 parents. All interviewed parents stated they did not have any
concern with facility.
Based on the information gathered from LPAs’ interviews, observation, and the record review, the preponderance of the evidence has not been met. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation of Staff handled child in a rough manner; therefore, the allegation is UNSUBSTANTIATED.
An exit interview was completed. The report was reviewed and discussed. Appeal
Rights were provided. The facility representative was provided a copy of their appeal
rights and their signature on this form acknowledges receipt of these rights. All appeals
must be in writing and received by the Licensing office within 15 business days. Any
proposed changes to the physical plant, including telephone number, shall be
immediately reported to the Department.
The facility representative was informed that the “Notice of Site Visit” must be posted for
30 consecutive days. Failure to post will result in Civil Penalties of $100.00. The “Notice
