Facility was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
In the areas that were evaluated, 15 Type B deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit.
Deficiencies observed are Physical Plant - Type B: 101174(d), Physical Plant - Type B: 101174(d)(2), Staff Records - Type B: 1596.8662(b)(1), Staff Records - Type B: 101216(g)(2), Staff Records - Type B: 101216(l)(1)(B), Staff Records - Type B: 101216.1(c)(1), Staff Records - Type B: 101217(a), Staff Records - Type B: 101217(a)(12), Children Records - Type B: 101220.1(g), Children Records - Type B: 101221(b)(6), Children Records - Type B: 101221(b)(7), Children Records - Type B: 101221(b)(8)(C), Children Records - Type B: 101221(d), Children Records - Type B: 101223(b)(1), and Reporting Requirements - Type B: 101212(b).
Exit interview conducted and report handed to Sub Teacher Kay Serafini. A notice of site visit was given and must remain posted for 30 days.
Appeal Rights and deficiencies were discussed. Sub Teacher was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.
Sub Teacher Kay Serafini refused to sign due to not knowing the regulations. A copy of this report dated 11/18/2022 was handed to Sub Teacher Kay Serafini.
