An Annual Random inspection was conducted at the facility by Licensing Program Analyst (LPA) Vivian Trinh. LPA met with licensee and census was taken. There were children which included 1 infant and 3 preschool age children in day care area were napping. Licensee was operating within the licensed capacity as specified on license. A review of the Facility Personnel Report Summary on this date 2/11/2025 indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Currently there are 3 adults including the licensee living in the home. Facility Day care hours are 5:30am-6pm, Monday through Friday.

During today’s inspection, LPA and licensee toured the inside and outside areas identified in the facility sketch as accessible to child care children. Off limits areas are made inaccessible by means of locked doors. This is a one story home which consists of 4 bedrooms, 2 bathrooms, kitchen and living room. Per licensee the bedrooms and other restroom are off limits to the children. The areas designated for the children are the living room, restroom next to the kitchen, the school age children use the kitchen for meal time and front yard for play-time. The front yard was fenced and observed to be free of hazards with sufficient toys. All areas identified on the facility sketch that children use, were inspected for safety, comfort, cleanliness, telephone service, ventilation and heating is available in the home. There are working carbon monoxide, smoke detector, and fire extinguishers in the home that meet statutory and State Fire Marshall standards. Detergents, cleaning compounds, medicines, and other items which could pose a danger if readily available to children were stored inaccessible to children. Licensee stated there are no firearms and/or other dangerous weapons in the facility and none were observed during today's inspections. There are no fireplace and/or an open-faced heater in the home. The home has age-appropriate toys for the ages served. The Licensee is aware that each FCCH shall conduct fire drills and disaster drills at least once every six months.

Safety precautions shall include but not limited to: (9) Each family child care home shall have a written disaster plan of action prepared on a form approved by the Department. All children, age and ability permitting, and the provider, the assistant provider, and other members of the household, shall be instructed in their duties under the disaster plan. As their age and ability permit, newly enrolled children shall be informed promptly of their duties as required in the plan. At 1:45 PM during document reviews, and interviewed, LPA observed the last firedrills was not document date and time, which posed a potential health, safety or personal rights risk to persons in care.

(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.

A record review shows that the last drill was not document date, and time. During today’s inspection LPA observed LPA verified there is a working telephone service (cellular service). Licensee stated they use the outdoor play area. There were no poisons or other items observed which could pose a danger to children or if they were observed, they were locked or inaccessible. There are no bodies of water, pets on the premises.

Garage will not be used for day-care. The applicant stated the garage will be off-limit.



Garage is to be utilized for limited activities, day-care children are not to napped or eat in the Garage.

The licensee does have a current roster of children in care of 4 children records for children present during the LPA inspection time. Children’s records for children present during LPA’s inspection were reviewed for a copy of the emergency information card that contains all the information specified by regulation (LIC 700), Immunization records, Affidavit Regarding Liability Insurance (LIC282), Consent for Emergency Medical Treatment (LIC627), Notification of Parent’s Rights (LIC995A) and found to be in compliance. Licensee stated, there is 1 infant under 24 months enrolled in the childcare. LPA reviewed LIC 9227 Individual Infant Sleeping Plan and napping log for infants.



The licensee and assistant’s Pediatric CPR/First Aid certification expires on 9/2025. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for staff were reviewed and within compliance.

Beginning March 31, 2018, H&S Code 1596.8662 requires all licensed providers and employees to complete mandated reporting training, and to renew the training every two years. At 1:45 pm, during staff file review and interview, Staff #1 and Staff #2 members did not have their mandated reporter training certificate expired 4/7/2024.

The licensee understands she must be present in the facility and must ensure children in care are always supervised. Children are not to be left alone in parked vehicles. When the licensee is temporarily absent from the facility, arrangements must be made for a qualified substitute adult to care and supervise children while absent. The substitute adult must have the required criminal record, child abuse index clearances, immunizations, Pediatric CPR/First Aid, and mandated reporter training.

LPA reviewed with licensee the following safe sleep best practices:
· Always place infants on their backs for sleeping
· Use only a tight-fitting sheet on the crib or play yard mattress
· Do not hang any items from the crib or above the crib
· Keep all items, including blankets, out of the crib or play yard
· Pacifiers may be used if they do not have items attached to them
· Infants should not be swaddled or have any items covering them while sleeping
· The temperature of the room should be comfortable enough for an adult to wear a t-shirt and not be too hot or too cold.

The facility was not in compliance and violations of the California Code of Regulations, Title 22, Division 12 were observed, discussed and cited at the time of the visit. Operation of a Family Child Care Home 102417 (g)(9)(A), Administration of child day care Licensing HSC 1596.8622(b)(1). (see LIC 809D).

Exit interview conducted and report was reviewed with the licensee Shipp, Shonnie. A notice of site visit was given and must remain posted for 30 days.

During the exit interview, the licensee, Shipp, Shonnie, confirmed that there are no Registered Sex
Offenders living in the facility and LPA completed the RSO profile in FAS.
