25 | This is an amendment of a licensing report dated 6/10/24. On 6/10/2024, Licensing Program Analyst A. Silva conducted a Case Management – Deficiencies due to deficiencies observed. An on-site Facility Personnel Report Summary review indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Census was 13 children including 4 infants under the age of one at the time of visit. According to Health and Safety Code 1597.465 Large family day care homes; number of children - A large family day care home may provide care for more than 12 children and up to and including 14 children, if all of the following conditions are met: (b) No more than three infants are cared for during any time when more than 12 children are being cared for. During today’s visit, the licensee and two assistants were carrying for 13 children, including four infants under the age of one year. A citation was issued (See 809D). The licensee was unable to show that any of the children present were enrolled in and attending kindergarten or elementary. The facility was not in compliance. Violations of Title 22 or Health and Safety Code were cited, including: 1597.465(b) A copy of the Capacity Regulations with rations in a large family child care home was provided. LPA A. Silva informed licensee that this licensing report dated 6/10/24 documents 1 “Type A” citation(s). Type A citation(s) must be posted for 30 consecutive days during the hours that children are in care as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. LPA A. Silva further informed licensee that a copy of this licensing report must be provided to parents or guardians of all clients currently enrolled by the next business day or by the next day the children are in care. a copy of this report must be provided to the parents or guardians of all newly enrolled clients for 12 months from the date of this report. a signed Acknowledgement of Receipt of Licensing Report (LIC 9224) form, or another written equivalent statement, must be placed in the child's file for verification of receipt of the report. Appeal Rights were provided and provided (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was provided and must remain posted for 30 days.
