25 | Licensing Program Analysts (LPAs) Valdez Santana and Meza conducted an unannounced case management inspection to confirm if excluded individual was residing in the home. Licensee was present along with 6 children (1 infant, 1 preschool and 4 school-age) at the time of the inspection. LPAs were provided a tour of the facility; day care areas and off-limit areas were inspected.
The Department was informed that the Licensee’s daughter, Ashley Quezada was associated to the facility, who has a life time exclusion from all community care licensing facilities. Licensee stated that her daughter, Ashley moved into the home in August 2024. Licensee stated her daughter is not present during daycare hours, which is from 6:30am to 6:00pm. Licensee understands that Ashley cannot be in the daycare areas during the hours of operation. LPAs provided Licensee with a copy of the Accusation, Decision and Order letters which state that Ashley is not allowed in Community Care Licensing Facilities.
The Decision and Order dated 12/21/2018 is excluding Ashley Quezada. The exclusion Prohibits employment presence and contact with clients of, any facility licensed by the Department. LPA Valdez Santana reviewed the letter with the licensee with the exclusion notice which states that an individual was excluded from any care facility licensed by the Department in the above order, pursuant to Health and Safety Code section 1596.8897, you are excluded from all care facilities licensed by the Department or certified by a licensed foster family agency and from holding the position of member of the board of directors, executive director, or office of the licensee of any facility licensed by The Department for the remainder of your life, unless you successfully petition for reinstatement or reduction of penalty pursuant the Government Code section 11522.
Exit interview was conducted. The Notice of Site Visit was posted. Licensee was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalty of $100. The licensee was provided a copy of their appeal rights and their signature on this form acknowledges receipt of these rights. |
