An Annual inspection was conducted in the facility by Licensing Program Analysts (LPA) Corral and LPA Tran. LPA Tran reviewed COVID-19 Prescreening Questionnaire with Licensee Lucia Castillo prior to entering the Facility. LPA Corral and Tran observed Licensee Lucia caring for 4 children which included I school age and 3 preschool age children. Licensee was observed to be operating within the licensed capacity as specified on license. Due to COVID 19 guidelines, LPA observed Licensee and Assistant wearing face masks, social distancing and following CDC and Dept of Public Health Guidelines. A review of the Facility Personnel Report Summary on conducted on 07/19/2021 indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Currently there are 4 adults living in the home which includes the Licensee, her Husband, her Daughter, her son in law and Licensee's 3 minor grandchildren. Facility operating hours are 5:30 am - 5:30pm, Monday through Friday and Saturdays 8am - 5 pm.

During today’s inspection, LPA Corral and Tran toured the inside and outside of the Facility with Licensee. The off limits areas of the Facility are 4 bedrooms and 1 bathroom which are made inaccessible by means of child safety door knobs. The child care areas available to children in care are the living room which is accessed through the main entrance of the home, the dining which is located to the right of the living room, the play room which is an addition to the home, along with the back yard. Licensee stated the children's primary area is the play room located passing the dining and living room. The kitchen is made inaccessible by the use of a child gate which is installed between the dining room and the kitchen entrance.

The home has a working carbon monoxide, smoke detector, and fire extinguisher which meets Statutory and State Fire Marshall standards. Detergents, cleaning compounds, medicines, and other items which could pose a danger if readily available to children were stored inaccessible to children in the kitchen above 5 feet high. Licensee stated there are no firearms or other dangerous weapons in the facility. A fireplace and an open-faced heater were not observed to be present during today's inspection.

The home has age appropriate toys for the ages served. During today’s inspection LPA observed 3 preschool aged children napping and 1 school age children doing activities with Assistant Sandra Castaneda. LPA verified there is a working telephone service which is the Licensee's cell phone. Licensee stated they do use outdoor play area. The outdoor play area was fenced and observed to be safe and free of hazards. The back yard has tricycles, play houses and arts and crafts material. There were no poisons or other items observed which could pose a danger to children in care. There are no bodies of water on the premises.

The licensee does have a current roster of children in care. Children’s records for children present during LPA’s inspection were reviewed by LPA Tran and found to be in compliance. Licensee conducted Disaster Drill on 06/15/2021. The Licensee's Pediatric CPR/First Aid certification was completed on 02/28/2021. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles and influenza for Licensee were reviewed by LPA Tran and found to be within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all licensed providers and employees to complete mandated reporting training, and to renew the training every two years. Licensee completed Mandated Reporter Training on 07/19/2021.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The licensee understands she must be present in the facility and must ensure children in care are always supervised. Licensee was informed children are not to be left alone in parked vehicles. When the licensee is temporarily absent from the facility, arrangements must be made for a qualified substitute adult to care and supervise children while absent. The substitute adult must have the required criminal record, child abuse index clearances, immunizations, Pediatric CPR/First Aid, and mandated reporter training.

CCLD website www.ccld.ca.gov was provided to licensee to access regulations, updates, and licensing forms. A copy of the California Department of Social Services Lead Information Brochure was explained and provided to the licensee. LPA reviewed with licensee the following safe sleep best practices:
- Always place infants on their backs for sleeping, only a tight-fitting sheet on the crib/play yard
- Do not hang items from the crib or above the crib, keep all items, including blankets out of the crib/play yard
- Pacifiers may be used if they do not have items attached to them
- Infants should not be swaddled or have any items covering them while sleeping
- The temperature of the room should be comfortable, not too hot or too cold

In the areas that were evaluated, No deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit.

An exit interview was conducted with Licensee in Spanish. Appeal Rights were provided and explained, signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals is to Regional Manager to the address listed above. The Notice of Site Visit was posted and discussed as required by H&S Code Sec. 1596.817. Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.00.
