During the investigation, LPA conducted observations and interviewed three (3) staff members and five (5) parents, LPA attempted to interview three (3) children who did not qualify and reviewed pertinent records, including LIC 9040 Children’s Roster recently party one (1) picture from Licensee and children in daycare from the one of the daycare child’s birthday.
During the visit on 04/24/25, LPAs Trinh and Navar observed five (5) preschool children supervised by 1 adult. During today’s visit 6/9/25, LPAs Vivian Trinh and LPA Kathy Trinh observed 5 children supervised by 2 adults. The facility was observed operating within the adult-child ratio requirement.
During record reviews, LPA reviewed the pictures from the birthday party the facility was celebrating, which indicated that the Licensee only has six (6) preschool children in daycare. Based on the Children's roster, five (5) children are enrolled in the facility.
During interviews, three (3) interviewed staff members stated that the maximum number of children in care is only five children in total. Five (5) interviewed parents did not raise any concerns regarding the above-mentioned allegation.
Based on the information gathered from LPA’s observations, interviews, and record reviews, there is insufficient evidence to corroborate the allegation the licensee was operating out of ratio. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the allegation did or did not occur in the day-care facility. Therefore, the listed allegation is UNSUBSTANTIATED.
An exit interview was completed. The report was reviewed and discussed with Licensee Vo, Thu Trung, (In Vietnamese). Appeal Rights were provided. Licensee Vo, Thu Trung was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. Failure to post will result in Civil Penalties of $100.00.
