On 11/4/24, at 2:00pm, Licensing Program Analyst (LPA) Anna Chan conducted a Plan of Correction (POC) inspection in response to a Type A violation issued on 11/01/24, 102417(g)(5)(A) Operation of A Family Child Care Home

LPA met with Licensee Pantea Akbari Pazooki. Licensee was caring for 10 children with assistant/spouse Behnam Beygzadeh present.

During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff and other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

The areas cited were re-inspected and found to be corrected. The pool gate is self-latching, the latch is 6” from the top of the fence. LPA observed the fence is strong enough to hold the weight of an adult in the home. Photos and videos taken during todays inspection.

No further action needed at this time. POC letter given, and correction has been received at this time.

Exit interview conducted and report was reviewed with Licensee Pantea Akbari Pazooki. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
