The purpose of the informal meeting was to discuss the following items:
102370 Criminal Record Clearance
Type ‘A’ deficiencies: (d) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing, or volunteering in a licensed facility: (1) Obtain a California clearance or a criminal record Exemption as required by the Department. On 03/10/22 LPA observed an uncleared assistant working in facility
102416.2(b)3(C)Reporting Requirements-Licensee failed to report an unusual incident. On 03/10/22 LPA observed that licensee failed to report when police made a visit
102423(a)3 and 4 Personal Rights -Children are to be treated with dignity in relationship with staff and other persons. On 05/24/22 LPA substantiated allegation that licensee handles children in an appropriate manner. LPA found that licensee had pulled on children's shirts. LPA found that licensee had forced a child to eat food by catching the food with spoon under chin and shoving food in mouth.
