A plan of correction inspection was conducted at the facility by Licensing Program Analyst (LPA) Castro and Mahnaz (Nancy) Malek. At 9:09 am . LPA met with licensee's assistant, Elizabeth Teran who was supervising 7 children of whom 2 were under 24 months old and 5 were over 2 years of age. LPAs met with Alexey Lopatin. The second assistant arrived to the facility at 9:15am. The other licensee, Simon Lopatin arrived to the facility at about 9:20am. Two additional children arrived later during the visit. Therefore the total of children are 9 children of whom 2 are under 24 months old with licensee and his two assistants. Licensee was operating within the licensed capacity as specified on license.
A review of the Facility Personnel Report Summary on this date indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.On 11/05/2024 the facility was in violation of staffing ratio and capacity under section 102416.5(d)(2)(a) of California Title 22 Regulations by having 2 children under 24 months old and 11 preschool children. A citation was issued on 11/05/2024. The facility sent a plan of correction to LPA on 11/06/2024 stating he is aware of the staffing ratio and he would comply with it at all times. On today’s inspection, LPA learned that the facility has taken care of the issue of staffing ratio by changing the children's' schedules. By this visit and LPA's observation the violation, which was cited on 11/05/2024, is cleared.
The licensee, Simon Lopatin was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00. The licensee, Simon Lopatin was provided with a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. The exit interview was conducted with licensee, Simon Lopatin.
