Licensee stated there are no firearms and/or other dangerous weapons in the facility and none were observed during today's inspections. The children's bathroom is located in the hallway. Licensee stated she escorted the children to the bathroom since it is located in the off-limits area.
LPA observed two (2) rocking chairs in the large living room that have been recalled according to the United Stated Consumer Product Safety Commission website; see LIC 809D for deficiency. The licensee removed the two rocking chairs to an off-limit area and provided LPA with a declaration statement to confirm that they understand recalled products are prohibited from licensed day care.
Also, the licensee did not have documentation of an emergency disaster drill from the last six months; see LIC 809D for deficiency.
The outdoor activity space was inspected for compliance. The space was enclosed by a fence at least five feet in height. The surface of the outdoor activity space was maintained and free of hazards. There were age-appropriate toys and materials for children in care. The fire pit is locked through the gas pipe line and inaccessible to children. There is a low high play structure with slide located on the grass area. There are age appropriate toys and equipment for children to play outside.
Licensee’s file was reviewed during the facility inspection on this date. Licensee possesses a current EMSA approved Pediatric CPR/First Aid certifications, which expires 10/22/2026. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a family childcare home if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against influenza, pertussis, and measles for licensee was reviewed and within compliance.
The licensee did not have a complete file or record for Staff 1 (S1) and Staff 2 (S2); see LIC 809D for deficiency. S1 did not have the following: Employee Rights (LIC 9052), Proof of immunization of measles, pertussis and Influenza, TB clearance or risk assessment, Statement Acknowledging Requirement to report Child Abuse (LIC 9108), and the Mandated Reporter Training Certificate. S2 did not have proof of immunization of measles, pertussis and Influenza; see LIC 809D for deficiency.
Beginning March 31, 2018, H&S Code 1596.8662 requires all licensed providers and employees to complete mandated reporting training and to renew the training every two years. Licensee’s mandated reporter training certificate expires 10/30/2026. (Continue to page 3)
