***All aspects of the complaint investigation, including this report, were completed by Licensing Program Analyst (LPA) Vivian Trinh. LPA Soo Jin Jung assisted in the delivery of this report on 1/8/2026.

On 1/8/2026, Licensing Program Analyst (LPA) S. Jung conducted a subsequent visit to deliver the findings of the complaint initiated on 10/15/2025. LPA met with Licensee Madrid, Sheri, who led LPA on a tour of the facility. At the time of the visit, LPA observed a total of one (1) infant and three (3) preschool children in the living room. LPA informed the licensee of the purpose of the visit.

On 10/15/2025, LPA Trinh met with Licensee Madrid, Sheri, who led LPA on a tour of the facility. At the time of the visit, LPA observed a total of one (1) infant and two (2) preschool children were watching an iPad in the living room. LPA informed the licensee of the purpose of the visit.

A review of the Facility Personnel Report Summary conducted on this date, 1/8/26, indicates all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions.

LPA Trinh conducted a physical plant inspection, interviewed the licensee and one (1) adult, and obtained pertinent documents.

During the interview, the LPA interviewed Staff 1 (S1) and Adult 1 (A1) and obtained declarations: Both S1 and A1 stated that A1 is only driving their own sibling with S1 in the car. Additionally, both S1 and A1 stated that S1 and Adult 2 (A2) are the only people who pick up daycare children from school. The LPA attempted to interview A2, but A2 refused to provide any information. Based on the record review, A2 meets all the requirements to pick up daycare children.

Based on the information gathered from the LPAs' observation, interview, and record reviews, there is insufficient evidence to corroborate the allegation: Licensee allows an unlicensed driver to provide transportation to children in care. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the allegation did or did not occur in the day-care facility, therefore the allegation is UNSUBSTANTIATED.

An exit interview was completed. The report was reviewed and discussed. Appeal Rights were provided. The facility representative was provided a copy of their appeal rights and their signature on this form acknowledges receipt of these rights. The Licensee was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. Failure to post will result in Civil Penalties of $100.00.
