A review of the Facility Personnel Report Summary conducted on this date, 1/8/26, indicates all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions.
LPA Trinh conducted a physical plant inspection, interviewed the licensee and one (1) adult, and obtained pertinent documents.
During the interview, the LPA interviewed Staff 1 (S1) and Adult 1 (A1) and obtained declarations: Both S1 and A1 stated that A1 is only driving their own sibling with S1 in the car. Additionally, both S1 and A1 stated that S1 and Adult 2 (A2) are the only people who pick up daycare children from school. The LPA attempted to interview A2, but A2 refused to provide any information. Based on the record review, A2 meets all the requirements to pick up daycare children.
Based on the information gathered from the LPAs' observation, interview, and record reviews, there is insufficient evidence to corroborate the allegation: Licensee allows an unlicensed driver to provide transportation to children in care. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the allegation did or did not occur in the day-care facility, therefore the allegation is UNSUBSTANTIATED.
An exit interview was completed. The report was reviewed and discussed. Appeal Rights were provided. The facility representative was provided a copy of their appeal rights and their signature on this form acknowledges receipt of these rights. The Licensee was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. Failure to post will result in Civil Penalties of $100.00.
