On 1/16/2025 at 2:00PM Licensing Program Analyst (LPA) K.Navar conducted an unannounced required annual inspection. Upon arrival LPA met with assistant Margarita Mendez who led LPA on a tour of facility. During tour of facility LPA walked through the living room to the dining room with dim lights. LPA then observed 1 infant asleep swaddled in a blanket bassinet with a pacifier that had a lanyard attached. Census was taken and LPA observed [2] infants and [4] preschool children. LPA then met with licensee Joselin Razo at 2:30PM.

An on-site Facility Personnel Report Summary review showed that all facility residents, staff, or other individuals who require background checks have received criminal record and child abuse index clearances or exemptions. [2] adults, including the licensee, and no minors are currently living in the home, according to the licensee and as shown in the Application LIC279 filed.

The LPA inspected the indoor FCCH area identified in the Facility Sketch LIC999 and areas accessible to clients. Off-limits areas were inaccessible to children in care by means of locks and safety gate at the time of inspection. The facility is a one-story home. The childcare area, which is accessed through the living room, consists of dining room, kitchen dining room, backyard, 1 bedroom, and bathroom. According to the licensee, the main childcare area is dining areas.

The facility equipped with working carbon monoxide and smoke detectors, working telephone service, and at least one fire extinguisher that meets statutory and State Fire Marshall standards. The licensee agrees to maintain telephone service in the home when children are in care. Hours of operation are 6:00AM-6:00PM. The licensee is aware that each FCCH shall conduct fire drills and disaster drills at least once every six months. A record review shows that the last drill was documented on 12/02/24. There is no fireplace in the facility.

The licensee stated that there are no firearms and/or other dangerous weapons in the facility; none were observed during the inspection. Detergents, cleaning compounds, medicines, and other items that could pose a danger if readily available were inaccessible to children in care. No poisons or other items that could pose a danger to children were observed during the inspection. Children nap on cushion mats. According to the licensee, beddings are stored individually and washed. The facility provides age-appropriate toys, play equipment, and materials for the children being served.

The licensee provides breakfast, lunch, and snacks to the children. At the time of inspection, food prep areas were clean and sanitary, and food was properly stored. Drinking water is available to children in care. Bottles are labeled with children’s names. The LPA observed that the facility’s floors, equipment, furniture, and children bathrooms were clean and in good repair.

The backyard was enclosed by a fence. The outdoor equipment and toys were in good repair and free of sharp edges. At the time of inspection, the surface of the outdoor activity area was well-maintained and free of any observable hazards. There were no bodies of water at the facility. There is a community pool a few minutes from facility that is fenced.

The LPA reviewed 2 files for staff who were present during the inspection. The licensee’s mandated reporter expires on 4/2025 and the EMSA Pediatric CPR/First Aid certification expires 4/25. The immunization records show that the licensee has been vaccinated against influenza, pertussis, and measles.

The licensee has a current roster for children enrolled. The LPA reviewed 5 children’s files of children who were present during the inspection. All reviewed files were in compliance and included completed copies of the LIC 700, LIC282, LIC627, LIC995A, and Immunization records. The roster shows there are 2 infants enrolled in the FCCH. LPA reviewed LIC 9227 Individual Infant Sleeping Plan and 15-minute check logs for 2 infants enrolled. The infant records were in compliance.

This facility provides Incidental Medical Services – IMS. LPA reviewed the storage of medication, equipment, and supplies and reviewed children’s, personnel, and administrative records. The licensee understands that he or she shall be present in the home and shall ensure that children in their care are supervised at all times. When circumstances require the licensee to be temporarily absent from the home, the licensee shall arrange for a qualified substitute adult to care for and supervise the children during his/her absence A qualified substitute adult is an adult that has criminal record and child abuse index clearances, immunization's, and current Pediatric CPR/First Aid and Mandated Reporter training]. Temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day, in accordance with Section 102417 of the California Code of Regulations. The licensee understands that children are not to be left alone in parked vehicles.

The licensee understands that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption prior to the initial presence in a licensed childcare facility. Violation of this requirement will result in a citation of a deficiency and civil penalties of one hundred dollars ($100) per violation per day for a maximum of five (5) days. Subsequent violations within a twelve (12) month period will result in a civil penalty of one hundred dollars ($100) per violation per day for a maximum of thirty (30) days in accordance with Section 1596.871 of the Health and Safety Code.

The licensee understands that a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year, in accordance with Section 1597.622 of the Health and Safety Code.

LPA reviewed the following safe sleep best practices with the licensee:
Always place infants 12 months and younger on their backs to sleep
Use only a tight-fitting sheet on the crib or play yard mattress.
Do not hang any items from the crib or above the crib.
Keep all items including blankets, pillows, and stuffed dolls out of the crib or play yard.
Pacifiers may be used only if they do not have items attached to them and there is no recall.
Infants should not be swaddled or have any items covering them while sleeping.
The temperature of the room should be comfortable enough for a lightly clothed adult.

LPA K. Navar informed licensee Joselin Razo that this licensing report dated 1/16/2025 documents 3 “Type A” citations. Type A citations must be posted for 30 consecutive days during the hours that children are in care as there is/are immediate risks to the health, safety, or personal rights of children in care. LPA K. Navar further informed licensee Joselin Razo that:

A copy of this licensing report must be provided to parents or guardians of all clients currently enrolled by the next business day or by the next day the children are in care.
A copy of this report must be provided to the parents or guardians of all newly enrolled clients for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224) form, or another written equivalent statement, must be placed in the child's file for verification of receipt of the report.

The LPA conducted an exit interview with Joselin Razo and reviewed the report with the licensee. LPA provided Safe Sleep for Your Baby Brochure. The “Notice of Site Visit” was posted, and the licensee is aware that it shall remain posted for 30 days.
