25 | On September 26, 2025, an unannounced annual random inspection was conducted at the facility by Licensing Program Analyst (LPA) Long Pham. Licensee Klymishen Alicia is not home when LPA arrived at 9:00am. Licensee was present at the facility at 9:05am. At 9:10am, LPA observed 2 assistants care for 6 preschool children. The Licensee was operating within the licensed capacity as specified on license.
A review of the Facility Personnel Report Summary on this date indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Currently, according to the Licensee, there are 2 adults including the licensee residing in the facility and 1 minor. Facility Day care hours are 8:30am-5:00pm, Monday through Friday.
During today’s inspection, LPA and licensee toured the inside and outside areas of the facility sketch as accessible to childcare children. This is a two-story single-family home with 4 bedrooms, 5 bathrooms, living room, dining room, kitchen, laundry room, front yard (not fenced), backyard (fenced) and garage. Licensee stated the day care area consists of all level one, including 1 bedroom, two bathrooms, living room, dining room, and the backyard. Off-limit areas include all second level, including the three bedrooms, laundry room and the three bathroom, front yard, and the garage. LPA verified the childcare areas on the facility sketch and off-limits are made inaccessible by means of child-proof safety gate. The facility doesn’t have a fireplace. There is at least one working carbon monoxide, smoke detector, and fire extinguisher in the home that meet statutory and State Fire Marshall standards. Last fire/disaster drill documented on 06/11/2025. Detergents, cleaning compounds, medicines, and other items, which could pose a danger if readily available to children, were stored inaccessible to children. Licensee stated there are no firearms and/or other dangerous weapons in the facility, and none were observed during today's inspections.
